Join OSCIPA

Thank you for your interest in participating in the Oklahoma State Chiropractic Independent Physicians Association. There are two ways you can submit your membership application to OSCIPA. Most of the application can be completed and submitted entirely online. You must print and submit by fax, email, or postal mail only the documents that require your signature. These are listed under the Checklist tab in the Application section.

Or you can download and print the entire application, along with the documents in the Checklist and complete and return them by email or postal mail.

If the file opens in your browser go to your File menu and choose "Print". To save it to your computer go to your File menu and choose "Save As" then you can choose where you would like it to save to on your computer.

Membership Fees

Initial Credentialing Fee: There is no longer an enrollment fee to join OSCIPA. However there is a one-time credentialing fee of $175. A check or credit card payment for the credentialing fee must be included with your application.

Monthly Membership Fee for IPA Members: As of July 1, 2011 OSCIPA’s monthly IPA membership fee for most continuing members is $125.

Startup Membership Fee: As of July 1, 2011 the startup fee for new doctors is $75 per month. The criterion for this rate is determined by your state license date. If your license date falls within the last 3 calendar years, you may be eligible for the startup fee. The year that you received your license plus two additional calendar years, you will be assessed a fee of $75 per month. Continuing membership fees after the three year mark will be at the regular membership rate.

Exceptions: Any exceptions such as practicing gaps, underserved areas, moving from an associate position to start up your own practice, and any other identifiable situations will be reviewed by the OSCIPA Executive Director.

Note: All OSCIPA membership fees are used primarily for operational expenses, CE Seminars & educational expenses, communications & advertising expenses (Website & Eblasts), and legislative expenses (lobbyist).
